There’s a point in most people’s dark-spot story where the serums stop feeling like enough. You’ve been consistent for months, you’ve got the sunscreen habit, and the spots are still there — fainter maybe, but there. So you start looking at clinics.
This isn’t a post that tells you which procedure to book. Nobody can do that from the other side of a screen. It’s the explainer we wish existed before that first consultation: what each category does, what recovery involves, and the risk nobody leads with — these procedures can make pigmentation worse.

Before anything else: what type of pigment is it?
Treatments aren’t chosen off a menu. They’re chosen off a diagnosis, and the diagnosis is the part you can’t do yourself.
Pigment sits at different depths. Discoloration in the upper layers responds more predictably to surface treatment; pigment that has settled deeper is slower and more stubborn, and some never fully clears. A dermatologist examining your skin — often with a specialized light that shows depth — is making the call that decides whether a treatment helps or wastes your money. Then there’s the driver: sun damage, a healed breakout, hormones, friction, a medication. Chemical peels, superficial through medium
A peel applies an acid solution that controls how much of the skin’s surface sheds, so newer, more evenly pigmented skin comes up underneath. Depth is the whole story. Superficial peels work in the outermost layers — gentlest of the options, typically a series rather than one visit, recovery usually described as mild flaking and pinkness for a few days, though that varies with the acid, its concentration, and how long it sits. Medium-depth peels reach further: visible peeling, real downtime, a stretch where you look worse before you look better.
The honest limitation: peels address pigment near the surface, and deeper pigment often doesn’t come along for the ride. They’re also inflammatory by design — a peel taken too deep for the skin it’s on can leave a new pigment problem where the old one was.
Lasers and light, described by what they actually do
Mechanism categories, not brand names. Marketing is aggressive, machines get rebranded, and the box matters far less than who’s holding it.
Pigment-targeting lasers. Energy tuned to be absorbed preferentially by melanin, breaking pigment up so the body can clear it; some use extremely short pulses meant to shatter it with minimal heat. Recovery gets called short, but the pigment doesn’t vanish that day — it clears over following weeks, and multiple sessions are typical.
Intense pulsed light. Not technically a laser. IPL uses a broad spectrum of light rather than a single wavelength, generally for sun-related discoloration and redness together, and treated spots commonly darken before they flake away. It gets called gentle, which misleads in one specific way: it isn’t selective about which pigment it heats, so it’s generally considered unsuitable for deeper skin tones and a poor choice for melasma.
Resurfacing devices. Controlled injury in a pattern to trigger remodeling, across the whole surface or in microscopic columns. Texture tools more than pigment tools, at the high end for downtime and risk.
All three share one caveat: a laser removes existing pigment. It does nothing to the reason your skin made it.
Microneedling, with and without radiofrequency
Microneedling creates controlled micro-injuries with fine needles to prompt a repair response; radiofrequency microneedling adds heat through the needle tips. Its reputation here comes from being lower-heat than laser, which in theory makes it gentler on skin that reacts badly to thermal energy. Some clinicians use it for melasma for that reason, and small studies have looked at it with mixed, modest results. Stay clear-eyed about two things: radiofrequency adds heat, and heat is what pigmentation-prone skin is most sensitive to. And at-home rollers are a different product — shallower, and mainly a risk of irritation and infection.
The route people skip: prescription topicals
The lowest-intervention option is routinely the one nobody considers, because it isn’t exciting.
A dermatologist has access to prescription treatments — topical retinoids, combination creams, hydroquinone — well above anything sold at a counter. They’re used under supervision for defined periods, because they carry real side effects and real rules about duration. Hydroquinone in particular is regulated differently country to country, and unregulated imported creams sold as skin lighteners are a documented hazard, including mercury contamination. Prescription strength is a doctor conversation, not a purchase.
For plenty of people, months of a well-chosen prescription routine plus rigorous sun protection makes a device unnecessary. Slower — also cheaper, lower-risk, and no chance of leaving you darker than you started.
The part the brochure doesn’t lead with
Heat and inflammation are themselves triggers for pigment production, and every treatment above works by causing controlled injury. The treatment for hyperpigmentation is, mechanically, a hyperpigmentation trigger.
Calibrated correctly, the repair outruns the reaction and you improve. Calibrated wrong — wrong device, wrong settings, too aggressive, too soon after sun exposure, skin that was never a good candidate — you can end up with post-inflammatory hyperpigmentation darker, wider, and more stubborn than what you arrived with. Loss of pigment, leaving pale patches, is also possible, and can be permanent. So is scarring.
The risk isn’t distributed evenly. It’s well documented that deeper skin tones carry a meaningfully higher risk of post-inflammatory pigment change from energy devices and aggressive peels, because there’s more melanin available to react. The variable that predicts your outcome best isn’t the technology. It’s the provider’s specific, repeated experience treating your skin tone. A modest device in cautious, experienced hands beats a flagship machine run by someone whose training covered one skin type. Ask directly — a provider who gets defensive has answered it.
Nothing here is one-and-done
Most protocols run as a series weeks apart, plus maintenance, so judging after one visit isn’t fair to anyone. Strict sun avoidance before and after is part of the treatment, not a footnote to it — treating sun-exposed skin raises the odds of a bad outcome. And the driver doesn’t disappear: remove pigment without addressing what caused it and it can come back. Which is why melasma disappoints people who booked a laser expecting an ending. It’s generally described as chronic and relapsing, managed rather than cured, and some treatments aggravate it.
Questions worth asking at the consultation
Take these in, written down. A good provider will be glad you asked.
- What type of pigmentation is this, and how do you know? If nobody names what they’re treating, nothing that follows is a plan.
- How many patients with my skin tone have you treated with this protocol? The question. Ask for a number, not a reassurance.
- What’s my risk of it coming back darker, and what would you do then? Listen for whether there’s a plan at all.
- Would you consider a test patch first? Common with energy devices. Raise it if it isn’t offered.
- What are the lower-intervention options, and why skip them? Surfaces whether prescription topicals were ever on the table.
- How many sessions, over what period, and what happens after? Get the whole course, maintenance included.
- What does recovery look like day by day? Ask for the unpleasant version, and for what would make you call them.
- Do I need to stop anything beforehand? Topicals, medications, and recent sun change the picture.
- Are you the one performing it? Consultation and treatment are sometimes different people with different training.
- Is there a reason not to do this at all? The most useful one here, and the answer tells you who you’re sitting with.
A genuinely good provider will sometimes tell you not to do it. That’s how you know you found one.

How we picked
One filter: would a dermatologist plausibly nod at this on your counter during a treatment course? Everything here is either sun protection or deliberately inert. We ruled out acids, retinoids, and brightening actives — what you can use around a procedure is the call of whoever performed it. Where this stops being a cosmetic decision
See a doctor rather than book a procedure if a spot is new, changing, growing, irregular in shape or color, itching, bleeding, or different from everything else on your skin. Treating that cosmetically can hide something that needed identifying — same if pigmentation appeared suddenly or started after a new medication.
None of this is self-diagnosable, either — you can’t tell melasma from other pigmentation by comparing yourself to photos online, which is much of why plans made without an exam go wrong. Will one laser session get rid of my dark spots?
Almost certainly not. Nearly everything here runs as a series, and results build in the weeks after each session.
Can any of this be done on tanned skin?
Providers generally want you out of the sun before and after, and recent tanning is a common reason to postpone — it raises the risk of the exact problem you came to fix.
One last thing
The pitch for in-office treatment is that it ends the problem. Sometimes it helps, sometimes it helps and the pigment drifts back, and sometimes it makes things worse — which one you get depends far more on the diagnosis and the hands than the machine.
Go to the consultation. Bring the questions. Be more suspicious of certainty than of caution, and take seriously the provider who suggests waiting, starting with a prescription, or doing nothing at all. That’s usually the one worth booking.
